By participating in any class, treatment, or service at Dumaguete Wellness, I hereby acknowledge and

agree to the following:

 

1. Assumption of Risk

I understand that wellness services, including but not limited to Pilates, Yoga, Massage, Reiki, Sound

Healing, Myotherapy, and other movement-based or therapeutic practices, may involve physical

and/or energetic activity that carries certain inherent risks. These risks may include, but are not limited

to, muscle strain, emotional responses, dizziness, physical injury, or other unforeseen effects.

I, therefore, voluntarily assume all such risks and accept full responsibility for my participation.

2. Health & Medical Clearance

​

I confirm that I am physically, mentally, and emotionally fit and capable of participating in the services

I choose. I have disclosed any and all relevant past or present health concerns, injuries, illnesses, or

medical conditions to the instructor or practitioner prior to my session, and I agree to promptly inform

them of any changes to my condition.

Pregnant participants confirm that they have consulted with and/or obtained clearance from their

healthcare provider prior to participating in prenatal classes, treatments, or wellness services.

If necessary, I have consulted with a qualified medical professional regarding my participation in these

services.

3. Release of Liability

​

I hereby release, waive, and discharge Dumaguete Wellness, including its owners, instructors,

therapists, practitioners, employees, and staff, from any and all liability, claims, demands, actions, or

causes of action arising out of or related to any injury, illness, loss, damage, or adverse effect resulting

from my participation in any class, treatment, or service.

I understand and acknowledge that my participation is voluntary and undertaken entirely at my own

risk.

4. Responsibility & Conduct

​

• I agree to listen to my body and immediately communicate any discomfort, pain, or concern

• to my instructor, practitioner, or therapist.

• I understand that I may stop or discontinue a session at any time and for any reason.

• I agree to treat staff, fellow clients, equipment, and the premises with respect and care.

• I agree to comply with all safety guidelines, studio policies, and instructions provided by the staff.

5. Cancellations & No-Shows

​

I acknowledge that cancellations must be made at least twenty-four (24) hours in advance to avoid

applicable charges or forfeiture of a session.

 

6. Equipment & Property

​

If I use any of the studio’s equipment, facility, or property, I hereby agree to handle all equipment,

facility, or property responsibly and with utmost care. I understand that I may be held financially liable

for any damages resulting from the failure to follow usage instructions, negligent usage, or misuse.

 

7. Agreement

 

By checking the agreement box during booking, signing this waiver, and participating in any service

or activity at Dumaguete Wellness, I hereby confirm that I have read, understood, and voluntarily

agreed to all the terms and conditions stated above.

I hereby acknowledge that I am participating voluntarily and entirely at my own risk.
